Reference: Nguyen, Van C. (1976). Health Planning and Programming in Nepal - Assignment Report (WHO Project: NEP SHS 005), WHO. URL-http://apps.who.int/iris/ The lack of accurate and relevant information needed for planning and evaluation of health services in Nepal had already been emphasized during country health programming in 1974 and during the formulation of Health Information Service Programme in 1975. This report will attempt to give an overview of the existing organization of health information services and is not intended to deal with the recording and reporting system. The basic constituents of a health information system are: the socio-demographic data on the population, the indicators of the state of health of the people, measurements of the utilization of health services, statistics on the resources available, environmental data and information on outcome of treatment and preventive measures.
