A recent article in World Health Forum forcefully expressed the need for hospitals to be involved in primary health care and gave interesting examples of ways in which this has been done (1). Great scope exists for innovation, trial ans error, and the sharing of experiences. In this connection I describe here certain approaches that we have explored at Patan Hospital in Nepal. They have been shaped by geographical, historical, political, social, financial and other factors. When the hospital opened in 1982 it inherited many influences from the mission hospital it superseded. However, it was intended that the new hospital should be run independently by a board on which the government, mission and local community would be represented in approximately equal numbers.
